IDENTIFICATION OF THE DATA CONTROLLER

 

CENTRO MÉDICO DE ESPECIALIDADES DE CIUDAD JUÁREZ, S.A. DE C.V. (the "Medical Center"), acting as the Data Controller for the purposes of the Federal Law on the Protection of Personal Data Held by Private Parties and its regulatory provisions (the "Data Law"), with its registered address at AVENIDA DE LAS AMÉRICAS No. 201 NORTE, COLONIA MARGARITAS, POSTAL CODE 32300, CIUDAD JUÁREZ, CHIHUAHUA, MEXICO, hereby informs you that it will process the personal data collected from you in accordance with the following:

 

PURPOSES

To provide you with the medical care service you are requesting.

To include your data in our patient care databases.

To schedule your appointment, if necessary.

To compile your medical file or records related to the services you request.

To send you—via the email address you provide—images and results of your medical tests (both current and previous, if applicable).

To create a username and password for you, enabling you to view your Radiology, Molecular Imaging, or Clinical Laboratory test results, which will be sent to you via email.

To share your clinical information with your attending physicians and consulting specialists, provided you have authorized this when accessing the Service.

To share your data with third-party payers in general for the payment of services provided by Centro Médico de Especialidades de Ciudad Juárez, where applicable and authorized by you.

To issue invoices and collect payment for services rendered.

 

SECONDARY PURPOSES

Your personal contact data may be used to ask you to complete service satisfaction surveys. Additionally, your data may be processed for statistical purposes, for the improvement of care processes, and for academic and/or research purposes; in such cases, care will be taken to ensure that you are not personally identified. ADVERTISING PURPOSES

Your personal contact details (home address, telephone number, and/or email address) may be used to send you information regarding promotions and the features of services offered by the *Centro Médico de Especialidades de Ciudad Juárez* (Medical Specialties Center of Ciudad Juárez)—including, where applicable, information about its healthcare initiatives.

If you do not wish to receive such information or do not want your data used for any of these secondary purposes, please notify the Medical Center by sending an email to administracion@centromedicojrz.com.

PERSONAL DATA

To achieve the aforementioned purposes, the following personal data will be processed:

Identification data: Full name, age, date of birth, gender, marital status, nationality.

Contact data: Home address, telephone number, email address.

Data contained in your insurance policy.

FINANCIAL DATA

To achieve the aforementioned purposes, the following financial data will be processed: bank account details, credit or debit card details used to pay for services, and tax information. If payment for services is made via check, please be advised that the check may be transferred to an individual or legal entity with whom the *Centro Médico de Especialidades de Ciudad Juárez* has a contract for check processing and support services.

SENSITIVE PERSONAL DATA

To achieve the aforementioned purposes, the following sensitive personal data will be processed: current health status, as well as health information obtained through the services you request at this Medical Center; for certain services, it may also be necessary to obtain other sensitive data from you, such as your religion.

VIDEO SURVEILLANCE

Security video cameras are installed within the *Centro Médico de Especialidades de Ciudad Juárez* facilities; these may capture images and/or video of activities taking place in common areas, such as: hallways, elevators, waiting rooms, reception, the cashier area, the cafeteria, among others. This measure is taken for the safety of yourself and the individuals who visit or work at said facilities.

 

PRIVACY COMMITTEE

You may request to exercise your ARCO rights—which consist of access to your personal data, as well as its rectification, cancellation, or objection to its processing for specific purposes, and the revocation of consent previously granted for the processing of your personal data (to the extent permitted by law)—through the Privacy Committee. The Privacy Committee is responsible for processing your request and providing a response. This Committee will also handle any requests you make to limit the use or disclosure of your data.
