(the "Medical Center"), responsible for the purposes of the Federal Law on Protection of Personal Data Held by Private Parties and its regulatory provisions (the "Data Law"), with address at AVENIDA DE LAS AMÉRICAS No. 201 NORTE, COLONIA MARGARITAS, POSTAL CODE 32300, CIUDAD JUÁREZ, CHIHUAHUA, MÉXICO; informs you that it will process the personal data collected from you with the following:

 

PURPOSES

Provide you with the medical care you require in accordance with the Medical Services Provision Contract, as well as the policies, procedures, protocols and other institutional regulations of the Ciudad Juárez Specialty Medical Center.

Incorporate your data into our patient care databases.

Integrate your clinical record.

Share your data with your treating doctor and consulting doctors indicated by your treating doctor, who are independent professionals of the Medical Center and who have assumed responsibility for your diagnosis, prognosis and treatment in front of you.

Share information and documentation of the medical care provided, with third-party payers in general, for payment of the Medical Center's services.

Upload to the results consultation system the results of your Laboratory and/or Radiology and Imaging studies, which you can consult with the username and password that will be provided for this purpose and you can also receive them via email to the address you have provided.

To comply with the requirements of the competent authority.

 

SECONDARY PURPOSES

Use your data for statistical purposes, to improve care processes, academic and/or research, for which care will be taken to ensure that you cannot be identified.

Provide your family and friends who request it, information about the room number in which you are hospitalized.

Share with representatives or volunteers of your religious community, information about your name and the room number in which you are hospitalized, so that you can receive spiritual support during your stay at this Medical Center.

Share information and documentation of the medical care provided to you, for the purposes of paying the fees of your treating physician and interconsultants, either directly with said physicians or with the legal entity in charge of managing and collecting said medical fees.

 

ADVERTISING PURPOSES

Your personal contact information (address, telephone number and/or email) may be used to send you information about the promotions and characteristics of the services offered by the Specialty Medical Center of Ciudad Juárez, mainly, including, where appropriate, information about its healthcare projects.

VIDEO SURVEILLANCE

Inside the facilities of the Ciudad Juárez Specialty Medical Center there are security video cameras, which may capture images and/or video of the activities carried out in common areas for your safety and that of the people who visit us and work in said facilities.

 

PERSONAL DATA

To achieve the aforementioned purposes, the following personal data will be processed: full name, address, telephone number, email address, marital status, age, sex, nationality, date of birth, name, address and telephone number of a family member that you designate as a responsible family member and with whom we can communicate in case of emergency; If applicable, your policy number and other information related to the medical expenses insurance you have contracted. In some services, photographs or videos will also be taken that will be integrated into your clinical record, in order to keep a record of your evolution or treatment.

 

FINANCIAL DATA

To fulfill the aforementioned purposes, if necessary, the following financial data will be collected: bank account data and tax data. If you make payment for services by check, we inform you that it may be transferred to any natural or legal person with whom the Medical Center has entered into a contract for the provision of support services and check transactions.

 

SENSITIVE PERSONAL DATA

In order to be able to provide medical-hospital care and in accordance with applicable health legislation, you will be asked for the sensitive personal data required for this purpose: religion, current state of health, past and present illnesses, family history, symptoms, history.
